You can send in the order form that comes with your catalog to place your order. You can place your order online or by calling if the benefit period is coming to an end and you don’t think your order form will arrive in time. By going here, you can also view a copy of the OTC Catalog.

A prepaid card containing your OTC benefit allowance will be sent to you.

- Type in the Benefits Mastercard® Prepaid Card’s 16-digit card number and expiration date.
- Type the Last Name and Date of Birth that you gave your health plan when you enrolled in it.
